What is SpO₂ Scale 2 and when do I use it?

Scale 2 is used only for patients who have a documented prescribed SpO₂ target of 88–92% due to confirmed hypercapnic respiratory failure (e.g. COPD, kyphoscoliosis). Do not apply it routinely.

Why does a single score of 3 trigger medium risk?

NEWS2 is designed so that any critically abnormal single parameter — even if the total is low — triggers an urgent response. This protects against patients with one severely abnormal reading being missed.

How often should NEWS2 be scored?

At minimum 12-hourly for score 0. Increase to 4–6 hourly for scores 1–4, and continuously for scores ≥5 or during any MET/RRT response.
